Fax your order form to the number listed on the form. You will be contacted within 24 hours to confirm your order.
By Email:
Send an email with a scan of your completed order form to info@annapurnanaturopathic.com. You will be contacted within 24 hours to confirm your order.
By Phone:
To order by phone, call 416-877-0474 with a detailed information including a) Type of test(s) you would like done; b) If you would like an interpretation; c) Your credit card type (visa, mc, etc.), number, CVV (number on the back), expiry date and name on the card; d) The shipping address that you would like the test kit shipped to. We will take your information and have your kit shipped to you once payment clears.
By Online Payment:
Online payment is not available at this time. We are in the process of setting up a secure online merchant account with PaymentTech. Check back soon for this option. In the meantime please order your kit via fax or phone. We will be glad to process your order. Thanks.
Step 3:
The kit(s) will be shipped to you via ExpressPost once payment clears. The kit will include a receipt and a set of instructions on how to perform the test.
